The Staff Burnout Scorecard
The Staff Burnout Scorecard is used to identify and rate "staff burnout" warning signs. Using this scorecard, you can quantify burnout symptoms according to (8) categories, to get a quick big picture perspective on the scope and specifics of staff or team performance problems
Instructions
- Describe assessment purpose in the Statement of Purpose field.
- Enter your assessment subject (team or individual staff member).
- Assign an assessment score to each of the (8) assessment variables. Score each on a scale of (1 - 4) as noted on the assessment form.
- Scores will automatically total. A scoring level of 17 and above is indicative of a noticeable to significant burnout problems. Lower scores (in the 9 - 16 point range) will indicate minor to moderate burnout.

Attendance - sudden, chronic lateness or an increase in absenteeism.
Productivity - decreases in the quantity of work accomplished over time.

Attention to Detail - small, but noticeable mistakes and omissions in work completed, marked by their unusual nature (i.e. things that have been done correctly in past are now slipping through the cracks).

Relationships - a reduction in socialization and team activities, marked by conflicts, arguments and withdrawal from group activities.
