The Policy Satisfaction Survey is a PDF fill-in form designed to solicit and collect feedback on the effectiveness of management policies (any type of policy you choose).
How do I use the Policy Satisfaction Survey?
Open the template and be prepared to fill in the following information:
Usage Notes: The form date is automatic.
- Enter the name of the person submitting the form, along with that person's contact email and phone.
- Describe the policy under review (multiple lines of text allowed).
- Select the most appropriate responses to each of the evaluation questions.
- Enter any notes and observations in the space provided (multiple lines of text allowed).
